Violations/comments: Observed front and back freezers with condensation or ice issue. Equipment and/or components are not maintained in good working order. The surfaces of cutting blocks or boards were severely scratched or scored and could not be effectively cleaned and sanitized. Observed baseboard and cabinet area damaged. The physical facilities are not being maintained in good repair. Walk in cooler floor entrance has water under the floor at threshold area. Fix this issue. The physical facilities are not being maintained in good repair. Went through code changes with manager. Also went through food security about outside freezers.
