Critical

Observed the presence of live insects, rodents, and other pests. Observed fruit-flies throughout facility. To prevent illness, the presence of insects, rodents, and other pests shall be controlled by: Routinely inspecting incoming shipments of food and supplies; routinely inspecting the premises for evidence of pests; Using methods, if pests are found, such as trapping devices or pesticides used according section (C), (K) and (L) of rule 3717-1-07.1 of this code; and eliminating harborage conditions. A follow-up inspection on Aug. 17 resulted in zero critical violations.

Non-critical

Facility did not have one employee with supervisory and management responsibility and the authority to direct and control food preparation and service with level two certification in food protection. At least one employee that has supervisory and management responsibility and the authority to direct and control food preparation and service shall obtain the level two certification in food protection according to rule 3701-21-25 of the Administrative Code.

Person-in-charge did not ensure that the facility has written procedures for employees to follow when responding to vomiting or diarrheal events. Person in charge shall ensure that the FSO or RFE have written procedures for employees to follow when responding to vomiting or diarrheal events that involved discharge onto surfaces.

Food storage containers are not properly labeled. Observed bulk items without a label. Working containers holding food or food ingredients that are removed from their original packages shall be identified with the common name of the food, except containers holding food that are readily and unmistakably recognized.

Observed improper storage of food items. Observed food on floor in walk-in-hotdogs. Food shall be protected from contamination by storing the food in a clean, dry location, where it is not exposed to splash, dust, or other contamination, and at least six inches above the floor.

Cold or hot holding equipment did not contain a thermometer, or the thermometer was not correctly placed. Thermometer is needed for turbo-air refrigerator and continental refrigerator. A properly designed temperature measuring device as specified in this rule shall be located in the warmest part of a cooling unit or the coolest part of a food warming unit.

Equipment and/or components are not maintained in good working order. Repair leaking condenser in walk-in. Equipment shall be maintained in a state of repair and condition that meets the requirements specified under rules 3717-1-04 and 3717-1-04.1 of this code.

Cleaned equipment and utensils, laundered linens, or single-service and single-use articles are not properly stored. Observed single service items on the floor. Cleaned equipment and utensils, laundered linens, single-service articles, and single-use articles shall be properly stored as described in this rule.

The floor and wall junctures are not properly coved or closed, or floor drains were not provided. Please fix coving by dishwasher. In FSO’s or RFE’s in which cleaning methods other than water flushing are used for cleaning floors, the floor and wall junctures shall be coved and closed to no larger than one thirty-second inch; where water flush cleaning methods are used, shall be provided with drains and be graded to drain, and the floor and wall junctures shall be coved and sealed.

Observed light bulbs or heat lamp that were not properly shielded or coated where required. Observed missing light shield by spaghetti cooker. Light bulbs shall be shielded, coated, or otherwise shatter-resistant in areas where there is exposed food, clean equipment, utensils, or linens, or unwrapped single-service or single-use articles and does not apply in areas used only for storing food in unopened packages. An infrared or other heat lamp shall be protected against breakage by a shield surrounding and extending beyond the bulb so that only the face of the bulb is exposed.

The ventilation system was not being properly cleaned or was creating a public health hazard. Clean top of fryer vent. Intake and exhaust air ducts shall be cleaned and filters changed so they are not a source of contamination. If vented to the outside, ventilation systems may not create a public health hazard or nuisance or unlawful discharge.  A follow-up inspection on Aug. 17 resulted in four non-critical violations.
