Violations/comments: Critical violation: person-in-charge did not ensure that employees are informed in a verifiable manner of their responsibility to report information about their health. There is no test kit available for measuring the concentration of the sanitizer. Repeat observation: equipment food-contact surfaces and utensils were not being sanitized. Was told that soda nozzles were only being cleaned with soap. They must be sanitized often with quat or bleach solutions. Repeat observation: observed no towels or hand drying device at the hand washing sink in back office. Observed no hand washing sign in the bathroom or above hand sink in back office area.
