Violations: Found several pans stacked together while wet (wet-nested).
Found microwave interior in need of cleaning, found small amounts of mold inside top of ice machine around ice shoot. (correcting)
Found sticker residue on front of cake reach-in and front of microwave.
Found broken microwave interior, broken top to shake machine.
Found several unlabeled bulk containers, shakers and squeeze bottles throughout the facility.
Found out of temp TCS food being held in ice (yogurt drink mix, 56F). Items in ice were discarded by PIC.
Found floors beneath all shelving in need of cleaning.
No thermometer at 3 compartment sink.
Comments: Reviewed handouts. PIC has signatures indicating employees have been notified of their responsibility to report foodborne illness; however, list only containers 5 of the illnesses. PIC will contact corporate office to have list expanded to include all 13 required by Ohio Food Code.
