VANDALIA — The Occupational Safety and Health Administration cited Electra Form Industries, 852 Scholz Drive, with 17 safety violations at the plastic injection mold maker’s facility, including failing to provide machine guarding, the federal agency announced Monday.
Proposed fines total $66,000. OSHA said it opened an investigation on Nov. 14 under the agency’s national emphasis program to prevent amputations.
“Failing to have proper machine guarding unnecessarily places workers at an increased risk for amputations and other injuries,” said Bill Wilkerson, OSHA’s area director in Cincinnati. “OSHA is committed to protecting workers on the job, especially when employers fail to do so.”
The company didn’t immediately return a call for comment Monday.
Fourteen violations the agency called serious involved implement lockout/tagout procedures for the energy sources of equipment, mount electrical outlets, provide personal protective equipment for employees working on electrical equipment and properly training workers performing voltage testing. A serious violation occurs when there is substantial probability that death or serious physical harm could result from a hazard about which the employer knew or should have known.
The company has 15 business days to comply, request an informal conference with OSHA’s area director, or contest the findings before the independent Occupational Safety and Health Review Commission.
OSHA said it is waiting to hear back from the company about an appeal or scheduling a conference to present its case.
