Greene County health inspections

The following are summaries of Ohio standard inspection reports on file from the Greene County Combined Health District, from Nov. 14-18. District information is available online at www.gcchd.org/inspections.

Pearl Bay Thai & Asian

133 E. Dayton-Yellow Springs Road, Fairborn

Date of inspection: Nov. 14

Violations: Found no bodily fluid clean up kit or clean up procedures available. Informed PIC and handouts were given.

Found no documentation that employees have received health information. Informed PIC and handouts were given.

Found several items in walkin cooler that were not date marked. Corrected.

Found container of roasted coconut without labeling.

Found wiping cloth stored in spray side of dishwasher sink.

Found buckets of soy sauce stored on floor of walkin and dry storage. Corrected.

Found sticker/residue on side of metal pans. Correcting.

Found lights out in bathrooms and in walkin cooler.

Found mop drying in cleaning bucket. Correcting.

Found light shield missing from light above cut prep table and light shield cracked over by dishwasher.

Comments: Correct all violations by next inspection. Any previously noted corrected violations must remain corrected.

Penn Station Restaurant

3800 Colonel Glenn Highway, Beavercreek

Date of inspection: Nov. 15

Violations: Found cleansers being stored together with food items (bulk sugar, baking soda, potatoes).

Found bags of onions and boxes of potatoes stored on the floor.

Found debris under stock shelving.

Found unclean lemonade dispenser, mold on ice machine backsplash, mold in and around ice chute at self-serve drink dispenser.

Found one employee’s clothing covered with an excessive amount of short, white hair.

No bodily fluids clean-up kit with written procedures.

No verification that employees have been notified of their responsibilities related to foodborne illness.

Comments: Reviewed handouts.

Domino’s Pizza

17 N. Allison Ave., Xenia

Date of inspection: Nov. 16

Violations: Found the cord to the electronic scale placed in the cheese soiled with food splash. Keep the cord cleaned and sanitized to prevent contamination.

Found wiping cloth solution mixed below the required level of quat sanitizer. 100 ppm found. PIC corrected and made new solution. Assure staff change out solution frequently.

Provide a written procedure and proper PPE along with a sanitizer to kill norovirus and train staff to assure requirements are met. Have completed before next inspection. Gave PIC handout to assist in writing procedure.

Found no sign off sheet for employees to show they have been trained and understand the requirements of the code. Gave PIC handout to assist.

Found employee coats stored at the prep table where contamination could occur. PIC corrected.

Found no handwashing signage in the employees restroom PIC corrected.

Found food stored adjacent to containers of chemicals on the s.s. table in the back. All chemicals shall be stored away from food surfaces to prevent contamination. PIC corrected.
