The following are summaries of state of Ohio standard inspection reports on file from the Greene County Combined Health District, from Aug. 15-19. District information is available online at www.gcchd.org/inspections.

Date of inspection: Aug. 16
Violations: Found the exterior of the walk in freezer in need of a detailed cleaning.
Found meatballs (46F) and other foods being held in the make table out of temperature. All TCS items were removed and discarded and service has been called to repair the unit. Call 937-374-5618 once the repairs have been made.
Found unlabeled water spray bottles and shakers.
Found the make table not working properly.
Found the wash water temperature to be 84F. Corrected.
Found stained ceiling tiles and holes in the walls. The new facility will be opening shortly.
Found areas under the equipment, in the walk in freezer, and walls in need of a detailed cleaning.
Found a jacket hanging over single use items. Corrected.
Found the mop stored in the mop bucket.
Found a wire rack lined with cardboard.
Comments: Provided new handouts and discussed new code changes and the following items are required to be completed/provided: a procedure for cleanup of vomiting/diarrheal incidents (viewable during inspections) and provision of a diarrhea/vomiting cleanup kit that is compliant with the new rules (see handout); a sign off sheet which indicates that employees have been informed of when they must report illness which is readily viewable during inspections; in addition to a level 1 certified PIC on all shifts. Each facility is required to provide a level 2 trained individual (see handout for information regarding how to secure this training).

Violations: Hot water in women’s restroom 129F.
Dish machine racks, fan guards in walk-in freezer, portable floor fan, door frame to sub make table, soufflé cup holder.
Metal pans wet nested (corrected).
Open drinking container in kitchen (corrected).
Holes in wall across from pizza oven – seal all holes so final finishes are smooth and easily cleanable.
Torn door seals on sub finish table – replace.
Souffle cup lids stored with food contact surfaces up, single-use lids stored in a cardboard box (corrected).
Spray bottle of sanitizer unlabeled (corrected).
Wiping cloth not stored in container of sanitizer.
