Date of inspection: June 24
Violations: Numerous noncommercial freezers in use. Equipment must be approved by a recognized testing agency. Food equipment that is acceptable for use in a FSO or RFE shall be approved by a recognized food equipment testing agency.
Leak under 3 comp sink. To prevent health hazards, a plumbing system shall be repaired according to the Ohio building code.
Ceiling tiles in rear have water damage. All ceiling tiles should be cleanable or vinyl coated. Areas of the floor in poor repair. The physical facilities shall be maintained in good repair.
Comments: Three noncritical items noted.

Violations: Two non commercial refrigeration units in use. Food equipment that is acceptable for use in a FSO or RFE shall be approved by a recognized food equipment testing agency.
Comments: Very good inspection! One non critical item noted.
775 W. Central Ave., Springboro
Date of inspection: June
Violations: Leak from 3 comp sink in the basement producing standing water. To prevent health hazards, a plumbing system shall be repaired according to the Ohio building code. Correct By: 07-Jul-2016.
Comments: Piece inside the smoothie machine has been replaced. A smoothie was made for me and nothing was seen in it. The leak in the basement is still active.
