Every school in Ohio is required each October to send the Department of Health vaccination information about their kindergartners, seventh graders, twelfth graders and any students who are new to the district.

The database below represents what was reported to the health department for the 2016-17, 2017-18 and 2018-19 school years. Search by county, city or school using the dropdown menus below.

Schools with fewer than 10 students in a given grade were redacted from the data by the Ohio Department of Health. Additionally, some schools did not turn in complete data every year.

Schools are listed by mailing address so some may be located by searching neighboring cities. Oakwood schools, for example, are listed under Dayton.
