Violations/comments: Observed raw chicken stored over green beans in walk in cooler. Food was not properly protected from contamination by separation, packaging and segregation. Observed measuring cup being stored in enclosed flour bins with handle immersed in food product. IN-use utensils are improperly stored. Observed food items in boxes improperly stored on floor of walk in freezer. Observed sushi in buffet where time as a public health control was being used without proper written procedures. Observed dented cans in dry stock. Observed a cooler meant for storing bottled beverages and packaged foods storing time/temperature controlled for safety foods. Equipment is not approved by a recognized testing agency. Observed non-commercial microwave, not approved by a recognized testing agency. Observed inadequate equipment for cooling or holding food. Observed condensation issue with condenser in walk in freezer. Equipment and/or components are not maintained in good working order. Observed cold holding buffet where the bottom of the cabinet flooring was water damaged, crack and contained mold. Equipment and/or components are not maintained in good working order. Walk in freezer gaskets in reach in cooler behind line where gaskets are coming apart and detaching from cooler. Observed a cracked/broken faucet at hand sink near three-compartment sink. The plumbing system was not maintained properly. Observed presence of insects, rodents and other pests.
