Violations/comments: Critical violation: person-in-charge could not verify Level II course completion. Person-in-charge stated that the person had passed the Level II exam and is waiting for certificate. Critical violation: person-in-charge did not ensure that employees are informed in a verifiable manner of their responsibility to report information about their health. Critical violation: observed ice door inside of ice shoot on drink machine to have mold residual on the outside. Repeat observation: observed no handwashing signs posted at hand washing sink(s) used by employees in front area, back of the facility and in the restrooms. Observed dust build-up on fan cases in walk-in cooler.
