Health Inspections: Papa John’s Pizza

Violations: Found Papa John's health policy does not list all 13 reportable illnesses. This is what staff are being trained on when working at the facility. Gave PIC handout as well as the above section code to assure the store adds the other illnesses and re-trains staff to assure they understand which illnesses are reportable.

Found the facility not utilizing the test strips to assure the proper concentration of quat sanitizer is being dispensed at the 3 compartment sink. Found the sanitizer bag empty upon inspection, so if staff are not checking it with the strips, dishware may have been sanitized without the proper concentration. PIC filled the sanitizer on site and was educated on how to use strips.

Found the hand sink by the prep table needs re-caulked to provide a tight seal to the wall and not allow moisture from getting behind it.

Found the sanitizer pumping at the 3 compartment sink was at too high a concentration (>400ppm). Discussed with PIC to dilute solution and utilize the test strips to assure proper concentration before using on dishware.

Repair the sanitizer dispenser to dispense the proper concentration.

Was unable to locate the Level II certificate for the person on site. Provide a copy of the Ohio certificate to me by e-mail or fax within 1 week to correct this violation. Keep a copy of the certificate on site.

Comments: Discussed with PIC that all food contact surfaces such as prep tables and racks shall be cleaned and sanitized every 4 hours or less to prevent bacterial growth. If a spray bottle of sanitizer is used, assure proper concentration and utilize single use paper towels to clean the surface, or utilize a container of sanitizer and keep the cloth stored in the sanitizer between uses to prevent bacterial growth.

Facility well maintained.
