Greene County Health Inspections

The following are summaries of state of Ohio standard inspection reports on file from the Greene County Combined Health District, from the week of Feb. 15-19. District information is available online at www.gcchd.org/inspections.

Lucky Dragon

126 Dayton St., Yellow Springs

Date of inspection: Feb. 17

Violations: Found employee washing hand with no soap (corrected).

Found employee washing hands in 3 compartment sink (corrected).

Found raw chicken stored adjacent to raw fish (corrected).

Found uncovered food in the walk in cooler.

Found unclean sanitizer solution in wiping cloth bucket (corrected).

Found food products stored on floor. Store all food products a minimum of six inches above the floor to reduce contamination of food products and to allow for easy floor cleaning.

Found food cooling completely covered in the walk in cooler (corrected).

Found numerous foods held with no date marking (corrected all).

Found interior of cooler unclean. Found exterior of make table unclean.

Found utensils stored facing multiple directions.

Found hand sink used to dump (corrected).

Found dumpster open (corrected).

McDonald’s

2776 Colonel Glenn Highway, Fairborn

Date of inspection: Feb. 17

Violations: In review of the time marking sheet from the previous day, it appears that during the evening that the items are not being properly time marked. (Ex./ shredded lettuce – last time marked for expiration at 4:04 pm on 2/17/16 for both trays; the leaf lettuce is marked for 4:03 pm on 2/17/16; the table tomatoes were marked for 4:01 pm on 2/17/16cheddar jack cheese was marked for 4:03 pm on 2/17/16; and the bacon strips were marked for 6:02 pm on 2/17/16). The management must ensure that the facility is operated within the guidelines of the food code at all times of operation. Addressed with PIC.

Found two open cups of soda in the facility, one on the dishwasher and another on the ice machine.

Found uncovered items in the walk in freezer. Correcting.

Found the ice scoop buried in ice in the ice machine. Corrected.

Found wiping cloths stored on counters throughout the facility.

Found a Sterlite home storage container being used as a salad storage container. Replace with commercial, food grade, storage containers.

Found a fryer basket with broken wire. Also found multiple cracked and broken lexan containers on the storage racks. Discarded.

Found food debris on the lexan containers stored on the clean items rack. Correcting.

Found the coffee filters left out on top of the machine unprotected, and some of the lids missing from the top of the cup dispenser (also with the cups overstocked).

Found utensils stored with handles facing in various directions on the clean item shelf.

Found no handwashing signage by the front handwashing sink. Found the areas under the shelving in the walk in cooler and freezer in need of cleaning, along with the fronts and sides of equipment.

Found a chemical spray bottle stored hanging on the end of the clean drainboard of the sanitizing dishwasher.

Found the shelving in the dry storage area rusted and must be repainted. This has been documented in previous inspection reports with a yet unfulfilled agreement to have the shelving units replaced by the end of 2015. Replace immediately in order to prevent further enforcement actions regarding this issue. An agreement has been made to have the shelving replaced by the end of the year.]

Comments: All violations must be corrected by re-inspection on or after 3/4/16.
