Violations/comments: Critical violation: person-in-charge did not ensure that employees are informed in a verifiable manner of their responsibility to report information about their health.
REPEAT OBSERVATION: Observed fishing bait being stored in cooler with packaged beverages. Observed paper towels standing by themselves on table next to toilet without protection from contamination. Observed no hand drying provision by the hand washing sink by the restroom entrance. Observed ice build-up underneath the cooler fan in the walk-in beer cooler dripping condensation on boxes and on floor. The physical facilities are not being maintained in good repair.
