Date of inspection: July 21
Violations/comments: Observed surface on prep sink, handles on sink and handles on paper towel dispenser with residual build up. Non-food contact surfaces of equipment are unclean. Observed grease trap that looks like it has not been cleaned, and also a grease trap smell in grease trap sink room. The plumbing system was not properly repaired. Baseboard near grease trap is in bad condition. Remove and reinstall new baseboard properly. The physical facilities are not being maintained in good repair. Observed caulking above three-compartment with moldy residual. Replace caulking properly. Observed the presence of many live gnats.
