Violations: Observed food that was not properly protected from contamination by separation, packaging, and segregation. Raw chicken stored over tortillas in prep cooler. To prevent contamination, food shall be protected from cross contamination by separating raw animal foods during storage, preparation, holding, and display from cooked ready-to-eat food as described in this rule.
Equipment food-contact surfaces or utensils are dirty. Pop nozzles had accumulation of soil. To prevent contamination, equipment food-contact surfaces and utensils shall be clean to sight and touch.
Observed improper storage of poisonous or toxic materials. Chemical cleaners stored next to deli tissue paper in dry storage. To prevent contamination, poisonous or toxic materials shall be stored so they cannot contaminate food, equipment, utensils, linens, or single-service or single-use articles by: 1) Separating the poisonous materials by spacing or partitioning; and 2) Locating the poisonous or toxic materials in an area that is not above food, equipment, utensils, linens, single-service articles or single-use articles.
A toilet room was not completely enclosed and/or did not have a tight-fitting, self-closing door. A toilet room located on the premises shall be completely enclosed and provided with a tight-fitting and self-closing door, except where located outside an FSO or RFE, or does not open directly into the food preparation area.
Observed an outer opening, window, or door without proper protection against the entry of insects and rodents. To prevent contamination, outer openings of an FSO or RFE shall be protected against the entry of insects and rodents as specified in this rule.
Observed no towels or hand drying device at the handwashing sink in restroom off of kitchen. To prevent contamination by hands, each handwashing sink or group of adjacent handwashing sinks shall be provided with: individual, disposable towels; a continuous towel system that supplies the user with a clean towel; a heated-air hand drying device; or a hand drying device that employs an air-knife system that delivers high velocity, pressurized air at ambient temperatures.
Observed soap and towels for handwashing at the food preparation sink, utensil washing sink, or the service sink. A sink used for food preparation or utensil washing, or a service sink or curbed cleaning facility used for the disposal of mop water or similar wastes, may not be provided with handwashing aids and devices required for a handwashing sink.

Violations: Observed nonfood-contact surfaces constructed of unapproved materials. Shelving used for soda syrup boxes is made of unfinished plywood. Nonfood-contact surfaces of equipment that are exposed to splash, spillage, or other food soiling or that require frequent cleaning shall be smooth and durable.
Comments: Facility is clean. All temperatures within required ranges Employees knowledgeable in proper food handling procedures. Good.
Facility has signed employee health policies available for review. Discussed Level 2 certification process for PIC.
