The woman was born in 1952. The Ombudsman explained that adoption records finalized before January 1964 are open to adopted persons with the proper identification. A special affidavit of Adopted Person is required, and the affidavit must be notarized. Two items of identification are also required, which can be a motor vehicle license, marriage record, social security card, military identification card or employee identification card. A $20 fee is also required.
The Ombudsman provided the woman a copy of the affidavit form, and she submitted her request.
