"Unfortunately, the 1520 Germantown Day-Mont location will be closing on December 24, 2017. All clients will need to be connected to their new provider by December 17, 2017. Please talk with your provider about where you would like to send your medical record," the note reads.

One patient, who did not want to be named, said it’s very short notice for people to find new providers.
Day-Mont is providing resources lists to patients.
"Please also talk to your primary care physician about medication maintenance. We apologize for the inconvenience, and appreciate you allowing us to participate in your care. Sincerely, Day-Mont Administration Team," the note finished out.
Officials with Day-Mont could not be reached for comment by phone or email.
The nonprofit operated at a $1.1 million loss in 2015, according to the latest annual financial disclosure. It had $5 million in revenue that year.
Ann Stevens, spokeswoman for Montgomery County Alcohol, Drug Addiction & Mental Health Services, said she did not have more information on Day-Mont closing and that the organization did not receive county funding.
She said there are other available options for treatment for behavioral health in the county listed on their website mcadamhs.org.
Services at Day-Mont, founded in 1972, included assessment, case management, counseling and medication management, according to ADAMHS.
Day-Mont also worked with clients referred from the local courts.
U.S. District Court Judge Walter H. Rice said it’s his understanding that Day-Mont is closing its mental health services but is transferring its addiction services to its satellite location, at 8145 N. Main St.
Rice said in the Dayton region, the mental health providers that are available are quality providers, but there are still access issues that will be exacerbated by the loss of Day-Mont’s mental health services.
He said it will leave less options and the options that are available might be a long distance for clients looking for a new provider.
“It’s just another sign that the people in West Dayton do seem to suffer the most when things like this happen,” he said.
