It happened along State Route 29 about a mile northwest of Sidney in Shelby County.
Investigators say 40 year-old Roger Gross drove left of center and into the path of this car with 4 teens.
Troopers say Gross was highly intoxicated.
His 34 year-old passenger went to Miami Valley Hospital.
The driver of the other car, 19 year-old Brad Vantilburgh from Sidney is in serious condition at Miami Valley Hospital.
Two other teen passengers are recovering at Miami Valley Hospital.
Gross has been booked on charges of aggravated vehicular assault, driving under a suspended license and driving left of center.
