Violations: Facility was not able to show that staff has been trained on the symptoms and reportable illnesses as required by the food code. Gave PIC a handout to assist.

Found whipped butter stored at room temperature. PIC discarded. The policy is to replace the frozen container they are stored in every 2 hours. This was not done.

Found ready to eat TCS foods and beverage mixes not date marked with a 7 day or less use by date. PIC had staff correcting upon inspection. Found 2 trays of burritos date marked to be used by 4/3/17. PIC discussed with employee who was making the burritos and the stickers were not changed when he prepared the burritos today. Corrected.

Found white cheddar cheese at room temperature. PIC could not find a procedure for how the white cheddar cheese is to be handled and whether it has a 4 hour hold or longer based on microbial testing methods. PIC discarded the white cheddar. Facility must keep the white cheddar

Found hashbrowns slacking at room temperature. PIC discarded.

Found many clean/white containers stored in the clean dish area that are scored from heat on the interior and the container can no longer come clean. These should not be used for holding food. PIC discarded.

Found no thermometer in the frappe refrigerator.

Found the handle to the microwave in need of repair. Found the front to the duct vent loose, allowing food to enter behind it. Repair to seal tightly.

Found the interior to the fry freezer not cleaned with frys and crumbs still in the unit. This must be removed of food and cleaned and sanitized every night as it is not staying at temperature. Found the interior to the microwave in need of cleaning, found the hot zone trays and cavities with food debris collected in and under bins. PIC had staff correct these upon inspection. Found a dirty wire tray stored with clean dishware. PIC sent back to dish machine.

Found countertops under the coffee, frappe, creamer equipment by drive thru in need of cleaning to remove spillage underneath. Found the filter over the fry freezer covered with dust and in need of cleaning.

Found a case of cups stored on the floor at the front line. PIC corrected.

Found the hand sink faucet leaking by the grill line.

Found the floor under the bulk coke station and refrigeration unit in the back in need of more detailed cleaning.

Facility is not documenting and retaining logs (after March 30) for the heat treat dispensing unit. This unit must also be emptied and completely cleaned and sanitized every 14 days to prevent bacterial growth. Facility may want to clean every 13 days to prevent the machine from locking out. A member of each shift must know how to obtain and document the logs.

Comments: Sanitizer buckets were overloaded with towels and the sanitizer was not evenly mixed for cloths to have the proper concentration of 50 – 100 ppm. Discussed with PIC to not place so many towels in bucket.
