Address: 22 E. Main St., Fairborn

Date of inspection: April 21

Violations: Found no level 2 food handler certification during inspection. PIC has stated class will be taken on May 15. Once class is completed email copies of certification to jredmond@gcph.info.

Found no bodily fluid clean up procedures available during inspection. Provided PIC with procedures from ODH and informed to keep kit that is next door at facility at all times.

Found a non commercial roaster under counter. Remove or replace with commercial when item breaks.

Found the following items in need of repair: cracked floor tile behind counter, chipped floor tile by refrigerator, and chipped tile by pool table.

Found the following areas in need of cleaning: under sink, back storage closet, and under-around equipment.

Found the shelves in refrigerator have chipped shelves clean/repaint so that item is smooth and easily cleanable.

Found numerous food containers around facility that are date marked but are missing a common label that tells what the item is. The date marking is not an issue on anything it is unnecessary to date mark items that are not TCS.

Found ice machine draining into large barrel in basement. Referred to plumbing department manager Kevin Ploutz.

Comments: Facility has non-commercial freezers in basement when these item break replace with commercial. Also where food products are stored on pallets unless employees are actively moving them food debris will draw pests. Recommend adding vertical shelves for ease of cleaning and reducing pests.
