Violations/comments: Critical violation: person-in-charge did not ensure that employees are informed in a verifiable manner of their responsibility to report information about their health. Critical violation: TCS foods were not being held at the proper temperature. Observed cheese being cold held at 55 F, corrected during inspection. Critical, repeat violation: refrigerated, ready-to-eat, TCS foods held refrigerated for more than 24 hours were not properly date marked. Observed Ready-To-Eat foods that were not properly date marked and were outside their original packaging (pepperoni, mushrooms, etc.), corrected during inspection. Repeat observation: equipment is not approved by a recognized testing agency. Microwave is currently not commercial and needs to be replaced. There is no test kit available for measuring the concentration of the sanitizer. Observed grease residual on handles to cupboards. Observed no hand washing sign(s) posted at hand washing sink(s) used by employees either in the restroom or near the sink right behind prep cooler. Repeat observation: observed cracked tile in front of coolers in back of food prep area.
