Cope Center

42 Arnold Place, Dayton

Date of inspection: Sept. 28

Violations: The multi-use eating utensils located in the sorters were observed with the food contact surfaces up. To prevent contamination of the food contact surface, ensure any eating or serving utensil are being stored inverted – with the handles up, or in drawer with the utensils all facing one way and with the handles accessible.

Comments: Ensure any food item held over 24 hours is date marked.

At the time of inspection, the PIC stated all food is delivered by a licensed central kitchen and served on site. Utensils and trays are cleaned and sanitized on site. Any leftover food item is discarded. PIC stated that there is no intention of preparing food in the kitchen.

The kitchen handwashing sink was properly stocked and an EPA regulated sanitizer was on hand. The reach-in cooler was observed holding food at 39F.

Papa Johns

6035 Brandt Pike, Huber Heights

Date of inspection: Sept. 28

Violations: Found the hood vents on the finished side of the oven with dust build up. Clean and maintain to limit potential contamination and fire hazards.

Comments: Observed diced ham properly date marked in the walk in cooler.

Handsinks are properly stocked with hot water, soap, and paper towels.

Doubleday’s Grill and Tavern

199 E. Alex-Bell Road, Centerville

Date of inspection: Sept. 28

Violations: Old food build up observed on shelf under the grill. Clean and maintain as needed to prevent old food build up.

Old food build up observed under and behind fryers, grill, center make line and coolers. Clean and maintain as needed to prevent old food build up.

Abner’s

2424 E. Third St., Dayton

Date of inspection: Sept. 29

Violations: Mashed potatoes and gravy observed at 120F and 114F on hot hold. Hot hold items must remain at 135F or higher in order to limit growth of pathogenic organisms. Items in violation discarded.

Processed tzatziki sauce observed without proper date marking in preparation refrigerator. Processed foods that are held for over 24 hours shall be date marked for disposal in no less than seven days after opening by the establishment.

Cutting board in kitchen has deep grooves and cuts that prevent proper cleaning. Cutting board is to be replaced or repaired so that the surface is smooth and cleanable.

Ajanta India Restaurant

3063 Woodman Drive, Kettering

Date of inspection: Sept. 29

Comments: This is a follow-up inspection to a complaint received by public health Dayton & Montgomery County about roaches infestation and chicken cooking and holding methods.

Multiple dead and alive roaches were observed below the fire alarm place, and by the prep sink.

Discussed with the person in charge (PIC) about:

- Ensuring the facility is clean

- Ensuring the structure is free of cracks, holes

- Ensuring the pest control operation is employed.

Discussed with the PIC about cold storage and cold holding temperatures for time/temperature controlled for safety (TCS) foods.

Warren County health inspections

Pizza Hut

6054 State Route 48, Suite M, Maineville

Date of inspection: Sept. 29

Violations: Food equipment surfaces are not cleaned at the required frequency. Brushes and containers used for crust finishers must be cleaned every 4 hours due to the nature use. The brush is in contact with TCS foods and is placed back into the container. Equipment food-contact surfaces and utensils shall be cleaned at a frequency specified in this rule.

Observed light bulbs or heat lamp that were not properly shielded or coated where required (in walk in freezer). Light bulbs shall be shielded, coated, or otherwise shatter-resistant in areas where there is exposed food, clean equipment, utensils, or linens, or unwrapped single-service or single-use articles and does not apply in areas used only for storing food in unopened packages. An infrared or other heat lamp shall be protected against breakage by a shield surrounding and extending beyond the bulb so that only the face of the bulb is exposed.

A toilet room was not completely enclosed and/or did not have a tight-fitting, self-closing door. A toilet room located on the premises shall be completely enclosed and provided with a tight-fitting and self-closing door, except where located outside an FSO or RFE, or does not open directly into the food preparation area.

Comments: Facility is clean and well maintained.

Please note that crust finishers (containers and brushes that are kept inside) must be cleaned every 4 hours due to the utensils being used in contact with TCS foods (pizza post-baking).

Wendy’s Old Fashioned Hamburgers

1155 Reading Road, Mason

Date of inspection: Sept. 29

Violations: TCS foods were not being held at the proper temperature. I found blue cheese crumbles cold holding at 54 degrees for over six hours on upper sandwich prep cooler. The manager voluntarily discarded. To prevent the growth of pathogens, except during preparation, cooking, or cooling, or when time is used as a public health control, TCS food shall be held at 135F or above (except that roasts cooked using time and temperature parameters in 3717-1-03.3 may be held at 130F), or at 41F or less.

The handwashing sink was not easily accessible. A hotel pan with clean utensils was blocking the use of handwashing sink. The hotel pan was removed and utensils were properly put away. To ensure frequent and proper handwashing, a handwashing sink shall be maintained so that it is accessible at all times for employee use.

(Corrected during inspection) Observed improper storage of poisonous or toxic materials. Observed restroom cleaner being stored above three compartment sink. The cleaner was removed from area and placed in proper chemical storage. To prevent contamination, poisonous or toxic materials shall be stored so they cannot contaminate food, equipment, utensils, linens, or single-service or single-use articles by: 1) Separating the poisonous materials by spacing or partitioning; and 2) Locating the poisonous or toxic materials in an area that is not above food, equipment, utensils, linens, single-service articles or single-use articles.

Equipment and/or components are not maintained in good working order. I found upper sandwich prep cooler near burger line not maintaining a few TCS foods (cut lettuce, diced/sliced tomatoes) within required cold holding parameters of 41 degrees or below. The temperature of these foods was 54 degrees. The cut lettuce and tomatoes were just placed out within the last hour according to management. All replacement TCS foods were placed on ice and will be monitored and rotated throughout shift until unit is repaired. Equipment shall be maintained in a state of repair and condition that meets the requirements specified under rules 3717-1-04 and 3717-1-04.1 of this code. Correct By: 01-Oct-2015.

Equipment components are not intact, tight or properly adjusted. Observed torn freezer seal on walk in cooler door. The manager stated a work order has been turned into corporate and the seal will be repaired before next routine inspection. Equipment components such as doors, seals, hinges, fasteners, and kick plates shall be kept intact, tight, and adjusted in accordance with manufacturer’s specifications.

Greene County health inspections

Date of inspection: Aug. 24

Violations: Dry food residues, dirt, dust and/or miscellaneous debris was found to be built up on the walls, floors, ceilings and/or other noted areas. Thoroughly clean the walls, floors, ceilings and/or other noted areas to remove the built up residues and maintain all such surfaces so that they are clean to sight and touch. This is to reduce contamination and potential insect and rodent attractants. Floor area under reach-in coolers and freezers (very dusty).

Individual disposable towels were not available and/or not installed in the towel dispenser at handwashing sink(s) within facility. Provide individual disposable towels within towel dispensers (or continuous towels, air dryers) at all handwashing sinks to ensure that all employees can properly dry their hands following washing. No disposable towels in ladies restroom (corrected).

Xenia Christian High school

1055 Wesley Ave., Xenia

Date of inspection: Aug. 25

Violations: Unclean food contact surfaces were found in the facility. All such surfaces must be thoroughly washed, rinsed, and sanitize to remove built up food debris in order to prevent bacterial growth and potential cross-contamination issues. Found the popcorn machine unclean (correcting).

The walls, floors, and/or ceilings were found to be in poor repair. Repair all damaged walls, floors, and/or ceiling so that they are smooth, durable and easily cleanable. Found cracked and peeling paint on the ceiling and walls in the concession stand. Scape all lost paint, and repaint.

Comments: License issued today.

Bentinos of Yellow Springs

107 Xenia Ave., Yellow Springs

Date of inspection: Aug. 21

Violations: Various aspects of the physical facilities were found to be in disrepair. Must repair the noted violations to maintain the facility in good operating condition and/or smooth and easily cleanable surfaces. Holes in walls at various locations – repair; door seal to walk-in cooler – replace; missing floor tile under pizza oven – replace.

Unclean food contact surfaces were found in the facility. All such surfaces must be thoroughly washed, rinsed, and sanitize to remove built up food debris in order to prevent bacterial growth and potential cross-contamination issues. Can opener, sharpener portion to slicer (correcting).

Unclean non-food contact surfaces were found in the facility. All such surfaces must be thoroughly cleaned to remove built up food debris, dirt, etc., in order to prevent potential cross-contamination issues. Can opener holster, external portions to pizza oven (greasy & dusty), shelving units in walk-in cooler, fan guard in walk-in cooler.

Found between-use utensils stored improperly. All between-use utensils must be stored: In the food with the handle stored out of the product; in running water (i.e., - dipperwell), in water that is maintained at 135F or more, or clean and dry on a clean work surface/container where the utensil and container/work surface are washed, rinsed and sanitized once every four hours or less. Correct this violation to reduce bacterial growth and/or cross contamination issues. Soufflé cup used to dispense cinnamon-sugar (corrected).

Comments: All above noted violations must be corrected prior to your next standard inspection.

Miami County and Piqua health inspections

Rolling Chassis

Address: 413 N. Main St., Piqua

Date of inspection: Oct. 6

Violations/comments: Observed food employee portioning out nacho cheese, lick fingers and continue to portion cheese. Do not lick fingers. Wash hands after contact with face, coughing, etc. Observed bulk food containers without labels. All food items must be labeled, unless sustainable to prevent misuse. Observed cold meat, cottage cheese, without dates. All ready to eat TCS foods must be dated with date opened or prepared. Discard after 7 days. Rolling pin was cracked and no longer smooth and easily cleanable. All food equipment must be smooth and easily cleaned. The slicer and the ice bin interior need to be cleaned. Back door open without screens. If a door is going to be left open for ventilation, it must be screened to prevent pests Missing ceiling tile above mixer. Replace to prevent contamination. Observed flies in kitchen. Take steps to reduce number of flies.

Bottle of sanitizer stored above prep unit. Do not store chemicals above exposed food.

Date of inspection: Oct. 6

Violations/comments: Cottage cheese open without a date. Must date all ready to eat TCS food with date opened. Discard after 7 days. The walk-in cooler and utensil containers need to be cleaned.

Sterling House of Piqua

Address: 1744 W. High St., Piqua

Date of inspection: Oct. 12

Violations/comments: Holding temperature in 3-door refrigerator above 111 degrees. Mostaccioli, 49 degrees; tuna salad, 47 degrees; cottage cheese, 45 degrees. Keep all TCS foods below 41 degrees at all times to prevent bacterial growth. Leftovers will be discarded and repair service called. The three-door refrigerator was leaking water/condensation. Repair.

Rally’s No. 1067

Address: 118 N. College St., Piqua

Date of inspection: Oct. 13

Violations/comments: Food containers put away wet. Must allow the items to completely dry before stacking. The floors had a build up of grease and needs to be cleaned.

China Moon

Address: 1055 S Miami St., West Milton

Date of inspection: Sept. 21

Violations/comments: Observed a large tub of food cooling in the walk-in cooler. Discussed with owners to cool in small amounts in separate pans to reach cooling time parameters. Observed some grease dripping from hood and pipes under hood. Clean more often to prevent accumulation.

Hong Kong Chinese Restaurant

Violations/comments: Observed lights on buffet not covered or scatter proof. Replace lights properly. Water condensation is dripping into bucket from cooler condenser. Water must be drained in an approved method. Resurface/replace cutting board, observed stained cutting board.
