Violations: Observed build up on wall behind prep table. The physical facilities shall be cleaned as often as necessary to keep them clean, and cleaning shall be done during periods when the least amount of food is exposed.
Alden Brown Elementary School

Date of inspection: Sept. 24
Violations: Observed peeling paint on the ceiling around pipe that runs above prep area. The physical facilities shall be maintained in good repair.
Comments: Good job on food handling and temperature control. One maintenance item noted.

Violations: Observed wiping cloth used for raw meat not stored in sanitizer as required to prevent spreading bacteria. Cloths that are in use for wiping food spills from tableware and carry-out containers shall be maintained dry and used for no other purpose; cloths in use for wiping counters and other equipment surfaces shall be held between uses in a chemical sanitizer, or laundered daily; cloths used with raw animal foods shall be kept separate from other cloths, dry cloths and solutions shall be free of food debris and visible soil; containers of sanitizer solutions shall be stored off the floor, and used in a manner that prevents contamination. Single-service articles, single-use articles or single-use disposable sanitizer wipes shall be used in accordance with EPA - approved manufacturer’s label use instructions
Comments: Facility is very clean and well organized. One item noted.
Gold Star Chili

Violations: Observed employee eating, drinking, or using tobacco in non-designated area. Several uncovered cups of personal drinks throughout facility. Drinks should be stored covered, or in a designated area away from equipment and single use articles. To ensure proper hygiene, an employee shall eat, drink, or use tobacco only in designated areas where the contamination of exposed food; clean equipment, utensils or linens; unwrapped single service or single use articles; or other items needing protection cannot result.
Fixed equipment did not appear to be properly sealed or spaced for cleaning. Seal behind prep sink is deteriorating, seal behind 3-compartment sink has soil accumulation and should be replaced. Equipment that is fixed shall be installed so that it is spaced to allow access for cleaning, from adjoining equipment, walls, and ceilings a distance of not more than one thirty-second inch or one millimeter; or sealed to adjoining equipment or walls, if the equipment is exposed to spillage or seepage; Counter-mounted equipment shall be installed to allow cleaning of the equipment and areas underneath and around the equipment as specified in this rule.
Comments: At time of inspection facility is clean and organized, all food checked at proper temperature. Good.

Violations: Observed multiple foods, inside sandwich prep cooler, being held cold above 41F. Ensure foods are held cold at or below 41F in order to limit the growth of bacteria.

PIC was informed to ensure foods are placed on ice or in another working cooler. PIC was also informed that it is recommended that food temperatures are monitored and recorded multiple times per day. PIC contacted maintenance for repair during inspection.
Observed left door gasket, on display cooler, in disrepair. Ensure cooler gasket is replaced so that it is intact, smooth, and easily cleanable.
Observed multiple spray bottles not labeled. Ensure spray bottles are labeled with name of contents so that possible contamination is prevented. Bottles hanging over three compartment sink.
Comments: Observed handsinks supplied with soap, hot water, and paper towels.
Observed acceptable sanitizer levels in sanitizer buckets (chlorine bleach) and in three compartment sink (quaternary ammonia).
Discussed cleaning of soda nozzles with PIC. Ensure nozzles are cleaned and maintained so that they are free of build-up. Soaking should not substitute wash, rinse, and sanitize.
PIC was informed that all coolers shall be provided a working thermometer and a food probing thermometer shall be available for use at all times.
A re-inspection shall occur 09/25/2015 or thereafter.
Rally’s

Date of inspection: Sept. 21
Violations: Observed door gaskets in ALL coolers with build-up of food residue. Ensure each door gasket is cleaned more frequently and maintained so that they are free of build-up.

PIC was informed to ensure foods are held cold at or below 41F, held hot at or above 135F, and re-heated to a minimum of 165.0F.

Observed chicken breast (grilled) not holding 135 degrees or greater to limit growth of bacteria. This food must maintain 135 degrees or greater hot holding. Fix issue immediately.

The carpet in dining room still needs replaced. Observed one nonworking freezer and one nonworking prep refrigerator outside behind facility. Dispose of unnecessary equipment.
