Observed raw chicken and meat next to and above containers of ready to eat foods in the case. This was corrected during inspection. TCS foods were not being held at the proper temperature. Observed meatballs and green beans in hot holding at 115 degrees F. This was corrected during inspection. Refrigerated, ready to eat TCS foods held refrigerated for more than 24 hours were not properly date marked. Observed cut watermelon with a date mark of 11/12. Manager stated he was dating for 10 days. This was corrected. Ready to eat TCS food that had been date marked was not properly discarded when required. Observed ham salad with a date of 10/22, which exceeds the seven days. Food discarded. Equipment food-contact surfaces and utensils were not being sanitized. Observed worker washing and rinsing dishes and not using sanitizer.

Violations/comments: Person in charge did not ensure that employees are informed in a verifiable manner of their responsibility to report information about their health, did not ensure facility has written procedure to follow when responding to vomiting or diarrhea events. Observed residue inside of both ice shoots of beverage dispenser. Non-food contact surfaces of equipment are dirty. Observed dust and debris gathering in metal grating outside of cold holding machine with sandwiches attached to bottom shelf. Cooking and/or baking equipment is not cleaned at the required frequency. Observed food residue and residue stains on the top of the microwave and above the microwave door gasket. The hand-washing sink was not easily accessible. Observed soda syrup spilling underneath rack of soda boxes in the back storage area of building beside the walk in cooler. Observed maintenance tools improperly stored which resulted in contamination. Observed brooms being stored in both back storage area and office area.

Top layer discarded. Observed ribs, 45 degrees, and chicken soup, 48 degrees in walk in cooler. Keep these TCS items below 41 degrees to prevent bacterial growth. Repair called in. Observed cracked food containers. When food containers are cracked the cracks provide areas for possible bacterial harborage. Should be smooth and easily cleanable. Discard. The following items need to be cleaned: shelves, hood, floors by dishwasher, grease trays and ceiling by dishwasher.

The following items need to be cleaned: dough roller, ceiling, floors, mixer, dishwasher, upright freezer, metal stem thermometer. Repair paint above pizza oven so smooth.
