Papa John’s

Address: 500 N. Main St., Piqua

Date of inspection: Oct. 19.

Violations/comments: Single service containers of blue cheese, ranch, cheese and pizza sauce kept at room temperature. These items were all marked “keep refrigerated.” They must be kept at 41 degrees or below to prevent bacterial growth. Items discarded. The pizza cutter was not being washed often enough. Food utensils must be washed at least once every four hours of continued use to prevent bacterial growth. The following items need to be cleaned: prep unit; keyboard at prep unit; walk in interior; tomato cutter; stored scoops; can opener; floor. The back door was propped open. This must be closed or properly screened to prevent pests.

Pizza Hut

Address: 1631 Covington Ave. Piqua

Date of inspection: Oct. 20

Violations/comments: The dishwasher was not dispensing enough sanitizer. Must have at least 50-ppm chlorine during sanitizing cycle. Repaired by staff. Monitor closely. The following items need to be cleaned: floors; ceilings; equipment exteriors; can opening; shelving; vegetable cutter; bowls and containers on shelves.

Subway

Address: 1125 E. Ash St. Piqua

Date of inspection: Oct. 24

Violations/comments: Chicken and turkey held above 41 degrees in prep line. Keep all TCS foods below 41 degrees at all times to prevent bacterial growth. Containers are too full. Only fill to line on containers. This is a repeat issue. Correct practice of overfilling. Top layer of food removed. Tomato slicer stored below cleaner on back shelves. Keep chemicals below food equipment to prevent potential contamination.

Tim Horton’s

Address: 635 W. Water St., Piqua

Date of inspection: Oct. 26

Violations/comments: Butter marked “keep refrigerated” at room temperature, 70 degrees. All TCS items must be kept below 41 degrees to prevent bacterial growth. Items discarded. Repeat issue. Provide thermometer for reach in cooler by drive through so temperature can be monitored. One inside currently is broken. Repeat. The following items need to be cleaned: prep unit interior; hot holding unit; tomato cutter.

McDonald’s West

Address: 1546 Covington Ave., Piqua

Date of inspection: Oct. 27

Violations/comments: Observed food employee handle trash with gloves while preparing food. Must wash hands and change gloves after working with trash before handling/preparing food. Employee washed hands and changed gloves. Leaf lettuce being held at room temperature for five hours. TCS foods can only be held at room temperature for a max of four hours. Discard after four hours to prevent bacterial growth. Program on label machine will be corrected.

American Legion Post 184

Address: 301 W. Water St., Piqua

Date of inspection: Oct. 27

Violations/comments: Observed ground beef stored above lettuce in upright refrigerator. Must store all raw meats below ready to eat items to prevent possible cross contamination. Items relocated. Packages of hamburger thawing on counter. Not a safe way to thaw. Only thaw items in refrigerator, under cold running draining water, while cooking or in microwave, if cooked immediately. Item placed in refrigerator.

Leaf & Vine

Address: 108 W. Main St., Troy

Date of inspection: Oct. 7

Violations/comments: Person in charge did not ensure that the facility has written procedures for employees to follow when responding to vomiting or diarrhea events. Refrigerated, ready to eat TCS foods held refrigerated for more than 24 hours were not properly date marked. Observed pre-cooked bacon to be in cooler without being date marked. Employee stated bacon was commercially pre-cooked. A food thermometer was not readily accessible. Observed that there was no thermometer in the cooler upstairs behind the bar also in the right hand cooler down in the basement. Equipment and components are not maintained in good working order. Observed a lot of condensation in the back of the inside of the right hand cooler in the basement, which led to the accumulation of a lot of ice. Equipment-food contact surfaces or utensils are dirty. Observed mold build up in the inside of the ice machine, towards the back of the machine. Total room door to the women’s bathroom needs to have a door closer on it.
