Violations/comments: Observed employee drinks in an upright cooler sitting on all different levels of shelving. Observed the facility using or serving raw fruit or vegetables without properly washing them. Observed apples sitting in tray in a display cooler for customer that was not washed and not wrapped or packaged before customer consumption. Observed bagels in display case without ingredient information available for customers. Bulk food display for customer self-service was not properly labeled. Non-food contact surfaces of equipment unclean. Observed cooking line equipment with residual food oil build up on surfaces. Observed ice machine shoot with unclean residual on the plastic piece that ice drops through. Food equipment surfaces are not cleaned at the required frequency. Observed a hand sink that was not secure to the wall with caulking. The plumbing system was not properly maintained. Observed the mop sink area that has damaged tiled wall. The physical facilities are not being maintained in good repair. Observed the mop sink room with debris/build up on floor. Observed a build up of dark residual on floor tile throughout the food service. Observed a dishwasher booster heater that is nonfunctional.
