Violations: Equipment components are not intact, tight or properly adjusted. Observed seals for freezer door in need of replacement causing a fluctuation of temperature for freezer from 15-32 degrees. Equipment components such as doors, seals, hinges, fasteners, and kick plates shall be kept intact, tight, and adjusted in accordance with manufacturer's specifications.
Observed accumulation of soil residue on nonfood-contact surfaces. Observed a build-up of debris on shelves in walk-in. Nonfood-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues.
The physical facilities are not being maintained in good repair. Observed several missing ceiling tiles within facility. The physical facilities shall be maintained in good repair.

Observed a build-up of dirt and debris. Observed a build up debris on floors throughout facility including walk-in refrigerator, and walk-in freezer. The physical facilities shall be cleaned as often as necessary to keep them clean, and cleaning shall be done during periods when the least amount of food is exposed.
Comments: Several improvements have been made. There is a possibility of remodel to facility in September.
