Violations/comments: No person in charge trained in level 1 food safety training was present. The training must be approved through the Ohio Department of Health. Person in charge did not ensure their employees are properly trained in food safety. The surfaces of cutting blocks or boards were severely scratched or scored and could not be effectively cleaned and sanitized. Felt handles on cooler or freezer equipment with food residual build up. Non-food contact surfaces of equipment unclean. Observed freezer floor with black residual build up.
