Violations: Person-in-charge did not ensure that the facility has written procedures for employees to follow when responding to vomiting or diarrheal events. Observed no vomit/diarrheal plan posted for facility. Person in charge shall ensure that the FSO or RFE have written procedures for employees to follow when responding to vomiting or diarrheal events that involved discharge onto surfaces.

Cold or hot holding equipment did not contain a thermometer, or the thermometer was not correctly placed. Observed no thermometer in pizza prep cooler. A properly designed temperature measuring device as specified in this rule shall be located in the warmest part of a cooling unit or the coolest part of a food warming unit.

The plumbing system was not properly repaired. Observed a slow draining hand washing sink next to pizza prep table. To prevent health hazards, a plumbing system shall be repaired according to the Ohio building code.

The plumbing system was not properly maintained. Observed disconnected drain pipe for prep sink causing the prep sink not to be used properly. A plumbing system shall be properly maintained.

The floor and wall junctures are not properly coved or closed, or floor drains were not provided in area where dry storage and chemical storage meet. In FSO’s or RFE’s in which cleaning methods other than water flushing are used for cleaning floors, the floor and wall junctures shall be coved and closed to no larger than one thirty-second inch; where water flush cleaning methods are used, shall be provided with drains and be graded to drain, and the floor and wall junctures shall be coved and sealed.

Observed no handwashing sign(s) posted at handwashing sink(s) used by employees in rear bathroom. A sign or poster that notified food employees to wash their hands shall be provided at all handwashing sinks used by food employees.

Observed no handwashing sign(s) posted at handwashing sink(s) used by employees at handsink next to pizza prep cooler. A sign or poster that notifies food employees to wash their hands shall be provided at all handwashing sinks used by food employees.

The physical facilities are not being maintained in good repair. Observed cracked floor going out to rear door. The physical facilities shall be maintained in good repair.

Comments: Hand washing signs on order. Please ensure that the hand sink is only used for hand washing. Please ensure prep sink is repaired and that no prepping is being done in three compartment sink. Please correct other violations by next inspection.
