Violations/comments: Time not marked on food items that time was being used as public health control. If time is going to be used to keep foods safe items must be marked with time prepared. Must be discarded after four hours.

Observed bulk food container under front counter without label. All food items not in original container must be labeled, unless unmistakable. Cup used as ice scoop. Provide scoop with handle. Provide working thermometer for front prep unit so its temperature can be monitored.

Observed broken food containers. Food containers must be smooth, durable and easily cleanable. Discard broken containers. Equipment not being washed properly. Must wash with soapy water, rinse with clear water then sanitize with 50-ppm chlorine. Rinse step being skipped.

The following items need to be cleaned: ice machine, prep unit, shelves in walk in cooler, sugar/salt containers, mixer, floors, walls, three-compartment sink. Several lights were out. Replace. Person in charge did not demonstrate adequate knowledge. Provide proper training. If violations are not corrected, action will be taken against food license.
