Butter on counter at room temperature, but marked “Keep Refrigerated.” All food items that are marked “Keep Refrigerated” must be held below 41 degrees at all times. The slicer was in need of cleaning.

Violations/comments: Observed food employee put on clean gloves and not wash hands before. Must wash hands before putting on clean gloves. Boxes of food on floor in walk-in freezer. Keep food at leave six inches off floor at all times to prevent contaminates.
Sour cream in dispenser is at 55 degrees. Keep all TCS foods below 41 degrees at all times to prevent bacterial growth. Discard. Observed open ready-to-eat TCS foods without dates, i.e., lettuce, cheese. All refrigerated TCS items must be marked with date opened. Discard after 7 days of being open. Doughnut boxes appear to be being re-used. These are single service items that are not allowed to be reused. Several pieces of equipment are broken and not working. Either repair or remove from food service.
Food employee was not washing utensils properly. Must wash with soapy water, rinse with clean water and then sanitize. Rinse step was being skipped. The following items need to be cleaned — sandwich prep unit, milkshake machine, pizza prep unit, three-door refrigerator and reach-in freezer. Dispensing utensils stored on counter. Store these items clean and dry or in the product with handle out or in dipper well. Bags of trash along building. Loose trash in storeroom. Properly dispense of. Remove all unused items from food service to aid cleaning.

Violations/comments: Clean behind cook line of residual build up frequently. Ex. hot water knob on hand sink next to restroom. Sanitizer level in three compartments is low. Use test strips to test levels. Ensure 300 ppm or greater (for quat). Notes: Remember to date label any open food (that is temperature controlled for safety or TCS food that is not used within 24 hours. Remember each person in charge per shift must be certified in level one food safety.
