1540 Miamisburg-Centerville Road, Dayton

Date of inspection: Oct. 19

Violations: Observed walk-in cooler door not self-closing. Ensure walk-in cooler door is repaired so that it fully self-closes preventing a rise in temperature of cooler and foods.

Observed white plastic shield in large ice machine with build-up of debris. Ensure shield is cleaned more frequently and maintained so that it is free of build-up.

Observed multiple non-food contact surfaces with build-up of food residue. Ensure non-food contact surfaces are cleaned frequently and maintained so that they are free of build-up.

- Around Stoelting slushy machine including tray.

- Tray of Stoelting ice cream machine.

- Metal drip tray below iced tea dispensers.

- All shelving inside walk-in cooler.

- Shelf near single service items and cole slaw (across from Stoelting ice cream machine).

- Ventilation filters above flat top grill.

Comments: Observed handsinks supplied with soap, hot water, and paper towels.

Observed acceptable levels of sanitizer (quaternary ammonia) in three compartment sink and in sanitizer buckets.

Burkey Family Restaurant

670 Shiloh Springs Road, Dayton

Date of inspection: Oct. 21

Violations: Observed what appears to be mold growth on opened applesauce. PIC discarded at the time of inspection.

Observed raw shrimp stored over ready-to-eat foods. Ensure raw product is beneath ready-to-eat foods and stored according to cooking temperatures.

Observed several bulk bin containers holding white substances without labels. Label containers so as to identify their contents.

Observed food items e.g., deviled eggs, broccoli salad, cucumber salad without datemarking. Ensure food items have proper datemarking so as to not exceed the 7 day requirement.

Observed sponges at sink area. Ensure sponges are not used to prevent contamination.

Observed employee set up three compartment sink up improperly. For manual washing, set up sink to wash, rinse, sanitize, in that order.

Observed cutting board surfaces with chips, cracks, and knife marks on surface. Resurface or replace cutting boards to ensure it is smooth and easily cleanable.

Observed hot temperature dishware machine’s rinse temperature to be 120F. Cease use and make repairs so as the rinse temperature reaches 180F.

Observed build-up inside the ice machine. Ensure ice machine is cleaned and sanitized to prevent build-up.

Observed can rack with debris build-up. Ensure rack is cleaned to prevent build-up.

Observed food build-up on shelving racks inside the walk-in cooler. Ensure shelving is cleaned and sanitized to prevent build-up.

Observed food debris on slicer that was not in use at time of inspection. Ensure proper cleaning of slicer is done after use.

Observed no covering on receptacle in women’s restroom. Provide a cover for receptacle in women’s restroom.

Observed light out above broasters. Must provide 50 foot candles of light in hot cooking areas.
