Johnson had walked away from the Cedarview Rehabilitation and Nursing Care facility, crossed over a fence and walked up an embankment into the roadway, troopers said.
He was struck shortly before 5:30 a.m. by a 2014 Honda Accord headed north on the Ohio 48 bypass between Ohio 123 and Monroe Road, according to a release.
The driver reported the crash, which remains under investigation.
