Violations: Only one stopper available at the three compartment sink. Ensure water may be held in the three compartment for washing, rinsing, and sanitizing of the equipment and utensils. Using cloths stopped into the drains to hold water temporary for manual warewashing.
Baseboard of the walk in refrigerator is no longer sealed to the base of the refrigerator. Ensure baseboard is maintained properly.
Area under the stationary equipment on the make line has build up of debris. Clean and then as needed to prevent excessive build up of debris.
Comet/bleach spray bottle being stored at the drain board of the preparation sink. Ensure hazardous chemicals are stored to prevent possible contamination of food or food contact surfaces. Manager moved the spray bottle to proper storage area during the inspection and corrected.
Comments: Discussed 2016 Ohio food code updates with the manager on duty.
Light bulbs are shatterproof bulbs.
Fryer oil changed every two days and filtered each day.
Refrigeration units on this date maintaining temperature controlled for safety foods at 41F or below.

Date of inspection: Oct. 7
Violations: Observed the following food items out of temperature range for cold holding: sliced tomatoes 46.2F, rice 50F, shredded cheese 45.8F, and rice 50F. Ensure all TCS foods are maintained at 41F or below to limit the growth of bacteria. PIC wrapped food items in plastic and placed them in ice during inspection.
Observed equipment being stored on top of one another while still drying. Ensure equipment and utensils are in a self-draining position that allows air drying prior to stacking.
Observed debris and what appeared to be insects on the floor at the bar between the cash register and beer tape corner. Ensure areas are clean and maintained on a frequent basis to prevent any potential source of contamination. PIC stated area will be cleaned during inspection.
Comments: Discussed proper drying methods for equipment and utensils with PIC and kitchen personnel.
Discussed with PIC putting ceiling tile back up by the three compartment sink.
Ensure banquet area handsink is supplied with soap and papertowels before serving.
Food items were properly stored and date marked during inspection.
Observed proper glove usage by kitchen personnel.
Discussed food code changes and PIC received a copy.

Violations: Observed employee not wash hands when changing task from preparing raw food to cooking ready to eat food at the time of inspection. All food service employees shall wash hands after switching tasks from working with raw meat to preparing ready to eat food. Discussed with manager.
Chicken is thawing at room temperature, chicken is 63F at the time of inspection. All food services shall thaw food by slacking as long as the temperature does not exceed 41F. Chicken was removed at the time of inspection.
Observed accumulation of food debris throughout the facility on multiple surfaces. All food services shall clean the facility at a frequency to prevent accumulation of debris and to prevent pest attraction. Discussed with the manager at the time of inspection.
Comments: Handwashing is available and adequately stocked at the time of inspection.
Food temperatures are satisfactory at the time of inspection.
