Holly’s Cafe & Carryout

Address: 112 N. Main St., Casstown

Date of inspection: Jan. 11

Violations/comments: Critical violation: person-in-charge did not ensure employees are informed in a verifiable manner of their responsibility to report information about their health. Person-in-charge did not ensure that the facility has written procedures for employees to follow when responding to vomiting or diarrheal events. Repeat observation: observed shelving in walk-in cooler and shelving above three-compartment sink with rust residual. Repeat observation: observed oil residual buildup on vent hood. Repeat observation: observed bottom of freezer with food residual build-up. Observed no towels or hand-drying device at the back handwashing sink(s).

Covington Care Center

Address: 75 Mote Drive, Covington

Date of inspection: Jan. 11

Violations/comments: Person-in-charge did not ensure that the facility has written procedures for employees to follow when responding to vomiting or diarrheal events. Observed food steamer doors that allow water to leak out of the steamer. Equipment and/or components are not maintained in good working order.

JJ’s Lunchbox

Address: 7031 S. County Road 25-A, Tipp City

Date of inspection: Jan. 12

Violations/comments: Critical violation: observed food that was not properly protected from contamination by separation, packaging and segregation. Observed eggs above other foods in cooler. Observed container of flour that was uncovered on the bottom shelf in dry storage. Observed improper storage of food items.

Tipp City Preschool Learning Center

Address: 1221 W. Main St., Tipp City

Date of inspection: Jan. 12

Violations/comments: There is no test kit available for measuring the concentration of the sanitizer.

Hot Head Burrito

Violations/comments: No violations at time of inspection.

Residence Inn

Address: 87 Troy Town Rd., Troy

Date of inspection: Jan. 12

Violations/comments: Observed ice scoop stored in holder with water and growth inside. Observed damp rag lying on counter beside microwave, corrected during inspection. Observed personal items sitting on top of ice machine in storage area. Critical, repeat violation: observed improper use of time as a public health control for up to four hours for shredded cheeses in self-service bar. Observed gasket inside of fridge falling apart on the left-hand door. Observed coffee stirrers being stored in self-service area where contamination can occur.

Ruby Tuesday

Address: 1780 W. Main St., Troy

Date of inspection: Jan. 13

Violations/comments: Observed wet cloths lying on food prep table and on hand washing sink. Repeat observation: the surfaces of cutting boards in front of prep coolers were scratched to the point that they need to be resurfaced/replaced.

Shell True North

Address: 1789 W. Main St., Troy

Date of inspection: Jan. 13

Violations/comments: Observed debris in cabinet underneath fountain pop machine. Observed no towels at the hand-washing sink, corrected during inspection. Temperatures were all within required ranges.

Violations/comments: Satisfactory at time of inspection.

Pizza Di Roma

Address: 987 E. Ash St., Piqua

Date of inspection: Feb. 2

Violations/comments: Provide test strips so that amount of sanitizer can be monitored; 50 ppm of chlorine minimum. The following items need to be cleaned: prep units, pop nozzle and pasta pot.

Hershey’s

Address: 987 E. Ash St., Piqua

Date of inspection: Feb. 2

Violations/comments: The cabinets need to be cleaned.

The ice cream scoops were stored used in dipper well that was not running. Utensils must be stored clean and dry; in product with handle extended or in a dipper well that is running and draining.

Charley’s Steakery

Address: 987 E. Ash St., Piqua

Date of inspection: Feb. 2

Violations/comments: The following items need to be cleaned: cabinet interiors, ice machine, food processor, slicers and tubs.

Several lights burned out in back prep area. Replace so there is adequate lighting. Remove items from the facility that are not needed for its operation to aid cleaning, i.e. painting supplies, helium tank, etc. Repeat issue.

Pretzel Time/Great American Cookie

Address: 987 E. Ash St., Piqua

Date of inspection: Feb. 2

Violations/comments: Provide hand soap for the hand sink in the restroom so hands can be washed as needed.

Piqua High School

Address: 1 Indian Trail, Piqua

Date of inspection: Feb. 3

Violations/comments: Observed yogurt cups and container of butter kept at room temperature that are marked “Keep Refrigerated.” All TCS foods must be kept below 41 degrees at all times to prevent bacterial growth. Discard these items and display safely in future. Observed signs of mice in storeroom. Continue to use traps, etc., to try and eliminate issue. New shelving ordered. Un-used items being removed. Continue to address and monitor until corrected.

Piqua Manor

Address: 1840 W. High St., Piqua

Date of inspection: Feb. 6

Violations/comments: The ceiling in the dish room is damaged and needs to be repaired.

KFC

Violations/comments: The hood and the interior of the walk-in need to be cleaned.

Violations/comments: Satisfactory at time of inspection.

Wendy’s Old Fashioned Hamburgers

Address: 1221 E. Ash St., Piqua

Date of inspection: Feb. 6

Violations/comments: Observed cracked food containers/lids. Food containers should be smooth, durable and intact. Cracks provide areas for bacterial harborage. The pop nozzle in front counter needs cleaned.
