With the support of Westminster Presbyterian Church and Jeremiah’s Letter, Public Health Dayton and Montgomery County has limited funds available to provide free birth certificates.
The following is required to get a free birth certificate:
- Completion of a Public Health application
- Must be 18 or older (head of households preferred)
- Proof of residence (if applicable)
- Telephone number
- Address prior to the storms
- Email address
To fill out an application and apply for a free birth certificate, visit St. Margaret’s Church, 5301 Free Pike, Trotwood, from 9 a.m. to 4 p.m. Tuesday or stop by the Harrison Twp. Community Center at 5945 N. Dixie Drive, Dayton, from 9 a.m. to 4 p.m. Wednesday.
