Comments: Found onions stored on floor.

Found dry food soils on lids on sandwich block, gaskets on reach-in refrigerator.

Found pans stacked wet above utensil sink.

Found paper goods stored under hand sinks.

Found loose baseboards around mop sink.

Found dry food soils on floors under walk-in cooler shelves.

Date of Inspection: Oct. 12

Violations: Equipment, Utensils and Linens.

Comments: Wash water temperature 90 degrees F.

Surfaces where food items are stored lined with paper towels.

(Unclean) interior portions of Bakers Pride Oven.

Electric kettle and toaster intended for household use only found in use (remove from facility).

Date of Inspection: Oct. 16

Comments: Found cracked door on hot holding chicken display.

Found dry food soils on reach-in freezer gasket.

Found missing caulk on prep sink.

Found debris on floor under ice machine.

Date of Inspection: Oct. 16

Found chicken above chill line in sandwich block at 49 degrees (corrected).

Found quat sanitizer level in rinse water at utensil sink above 400 PPM (correcting).

Found dry food soils on lid to rear sandwich block, on gaskets to and shelf under reach-in refrigerator, behind drive thru soft drink dispenser.

Found rear hand sink used as a dump sink.

Found residue on walk-in cooler floor under shelves.

Domino’s Pizza

1258 Kauffman Ave., Fairborn

Date of Inspection: Oct. 18

Violations: Management and Personnel; Food; Equipment, Utensils and Linens; Water, Plumbing and Waste; Physical Facilities; Poisonous or Toxic Materials.

Comments: Found an employee open drinking cup stored on a food storage shelf (corrected).

Found hand sink next to the 3 compartment sink filled with scratch pads.

Found no hand washing signs at any of the hand washing sinks except the front hand washing sink.

Found corner cove base tile damaged under the hand sink next to dough cooler and to the left of the prep table.

Found wet wiping cloth on the prep table.

Found spray bottle above the pizza sauce unlabeled (corrected).

Found a spray bottle of Windex stored in the food prep table (discarded).

Found 2 spray bottles of sanitizer stored with a sleeve of cup lids (corrected).

Found unclean shelf under pizza trays above the make table.

Found hood vents unclean.

Found pizza dough out on the rack above the make table not time marked (corrected).

Taco Bell #23365

Date of Inspection: Oct. 16

Comments: Found spray bottle of cleaner stored next to shelving of paper products (corrected).

Found lids for bottles unclean. Found a few soda nozzles at customer self-serve area unclean (corrected).

Found small holes in the wall above the prep sink, need filled.

Found floor under cook line unclean.

Found top shelf above the fryer area unclean (process of correcting).

Roosters Xenia

400 West Main St., Xenia

Date of Inspection: Oct. 16

Violations: Food; Equipment, Utensils and Linens; Physical Facilities.

Comments: Found wings at 47 degrees, pizza make table 46 degrees (correcting).

Found damaged gaskets on pizza make table.

Found damaged fry baskets in use.

Found dry food soils in utensil bins by dishwasher and in bar, behind line on salad block, on shelf above utensil sink, gaskets on make tables.

Found make up air vents in hood blocked and excessive heat in kitchen area (the makeup air provides adequate ventilation to keep from removing heated or air conditioned air from facility and helps to remove grease vapors from kitchen.)

Found debris on floors under equipment and shelves in walk in cooler, kitchen area and under ice bin in server’s area.
